Terms and Conditions
By registering my child with Raise Leaders Martial Arts, LLC ("Raise Leaders"), I acknowledge that I have read, understand, and agree to the following Terms and Conditions. These terms apply to all Raise Leaders programs, including but not limited to:
After-School Program
Martial Arts Classes
Summer Camp
Spring Camp
Fall Camp
Winter Camp
Day Camps
Special Events, Clinics, Workshops, Tournaments, Demonstrations and Off-Site Activities
1. Enrollment
Enrollment is subject to availability and completion of all required registration forms and fees. Raise Leaders reserves the right to refuse or discontinue enrollment if participation is determined to be unsafe or not in the best interest of the student or other participants.
2. Health and Medical Information
Parents or legal guardians agree to provide accurate medical information, including allergies, medications, medical conditions, dietary restrictions, and emergency contacts. Parents are responsible for notifying Raise Leaders immediately of any changes to their child's medical information.
3. Emergency Medical Authorization
In the event of an emergency and a parent or emergency contact cannot be reached, I authorize Raise Leaders staff to administer basic first aid and CPR, and to seek emergency medical treatment, including transportation by emergency personnel if deemed necessary. I understand that I am financially responsible for all medical expenses incurred.
4. Participation
I understand that my child will participate in physical activities including martial arts instruction, games, sports, outdoor activities, transportation, field trips, and recreational activities. I voluntarily permit my child to participate in all scheduled activities unless I provide written notice of any restrictions.
5. Transportation
I authorize Raise Leaders staff and approved drivers to transport my child to and from school, field trips, recreational activities, and other approved program destinations as part of participation in Raise Leaders programs.
6. Assumption of Risk
I understand that participation in martial arts, athletics, recreational activities, camps, transportation, playground activities, and field trips involves inherent risks, including but not limited to falls, collisions, equipment-related injuries, sprains, fractures, illness, and other unforeseen injuries. I voluntarily assume these risks on behalf of my child.
7. Release of Liability
To the fullest extent permitted by law, I release and hold harmless Raise Leaders Martial Arts, LLC, its owners, instructors, employees, volunteers, contractors, agents, and affiliates from any claims, liabilities, damages, losses, or expenses arising from my child's participation in any Raise Leaders program, except where prohibited by law or resulting from gross negligence or intentional misconduct.
8. Insurance
I understand that Raise Leaders does not provide medical or accident insurance for participants. I agree that maintaining adequate health insurance coverage for my child is my responsibility.
9. Behavior Expectations
Students are expected to demonstrate respectful behavior toward staff, instructors, volunteers, fellow students, and property. Raise Leaders reserves the right to suspend or terminate participation without refund for behavior that jeopardizes the safety or well-being of others or significantly disrupts program operations.
10. Tuition, Fees, and Payments
Registration fees, tuition, camp fees, and other program charges are due according to the selected payment schedule. Registration fees are non-refundable unless otherwise required by law. Returned payments may be subject to additional fees.
11. Attendance and Absences
Parents are responsible for notifying Raise Leaders of absences whenever possible. Tuition is based on enrollment rather than attendance, and missed classes or program days are generally not eligible for refunds or credits unless otherwise stated by Raise Leaders.
12. Photography and Media Release
Unless otherwise requested in writing, I grant Raise Leaders permission to photograph and record my child during participation in programs and to use such photographs, videos, or recordings for educational, promotional, advertising, website, and social media purposes without compensation.
13. Personal Property
Raise Leaders is not responsible for lost, stolen, or damaged personal belongings brought to any program or event.
14. Program Changes
Raise Leaders reserves the right to modify schedules, instructors, activities, transportation routes, locations, field trips, operating hours, policies, and program offerings when necessary due to weather, safety concerns, staffing, facility needs, or other unforeseen circumstances.
15. Communication
I agree to receive communications regarding enrollment, schedules, emergencies, billing, and program updates by phone call, email, and text message using the contact information provided during registration.
16. Agreement
I certify that I am the parent or legal guardian of the registered child and have the legal authority to execute this agreement. By signing electronically, I acknowledge that I have read, understood, and agree to these Terms and Conditions. My electronic signature shall have the same legal effect as a handwritten signature.
